The Sign of Ruin is obtained through the Blackstone Key DLC or the Digital Deluxe Edition of Dredge. You cannot get it while playing the base game. To get the Sign of Ruin, you need to purchase either the Dredge Digital Deluxe Edition or the Blackstone Key DLC, depending on your platform.
